Family Practice at Grace Health provides your family with complete medical care for every member of your family from infants to the oldest family members by board-certified family practitioners, nurse practitioners, physician assistants, and registered nurses. We offer a full range of services from physical examinations and annual checkups to managing chronic medical conditions, acute medical care and family planning. We focus on all of your health concerns and will coordinate care with specialists when necessary.
Fresh Food Pharmacy
Grace Health teamed up with South Michigan Food Bank and the Food Council of Michigan to offer Grace Health’s Fresh Food Pharmacy to support our patients who are diabetic, pre-diabetic, have high blood pressure and/or heart disease. The Fresh Food Pharmacy is a 6-month program that helps patients take control of their health, one meal at a time. During the program, patients meet with a dietician or a Healthy Lifestyles coach once a month to learn about healthy habits and gain support on their journey to better health. They will also visit Grace Health Food Pantry twice a month to select nutritious foods and recipes. To see if you qualify for Grace Health’s Fresh Food Pharmacy, talk with your provider today.
