Grace Health had an Operational Assessment in 2012 with the Health Resources and Services Administration (HRSA), an agency of the Department of Health and Human Services (DHHS). HRSA is the primary agency for improving access to healthcare services for people who are uninsured, isolated or medically vulnerable. Grace Health passed all 19 criteria. Our next review will be in 2017.
In 2011 Blue Cross and Blue Shield designated Grace Health as a “Patient-Centered Medical Home.” This recognized that Grace Health offers patient-focused care with a primary care physician being the medical care home for health care needs and continuity of care.
It also means that Grace Health offers a team of providers who are accountable for patients’ physical and mental health care needs, including prevention, wellness, acute care and chronic care. In addition, Grace Health coordinates patients’ care with additional services if that becomes necessary including specialty care, hospital stays, home health care, community services and other supports.
